Serratus Anterior

Patient position: supine, with the elbow
extended with the shoulder flexed to 90
degrees and internally rotated. Protract the
shoulder upward, toward the ceiling to project
the upper extremity anteriorly. Scapular
movement must be observed and the inferior
angle palpated to be sure the scapula is
abducting. The table s firm surface supports
the scapula so there will be no winging and
the pressure of the hand may give the false
impression of strength. Because substitution
for a weak serratus anterior occurs easily in
this position, the preferred test position is
seated.
Examiner pressure:
Against the patient s fist, transmitting the
pressure downward through the upper
extremity to the scapula to adduct the
scapula. Slight pressure may be applied to
the lateral border of the scapula as well.
