The influence of cytokines (IL-6, IL-8) on cardiac function in patients with recent myocardial infarction.

Proinflammatory cytokines are known to play an important role during myocardial infarction by initiating the acute-phase reaction.
The purpose of the study was to evaluate changes of serum IL-6 and IL-8 level in patients with recent myocardial infarction and to compare it with cardiac function during myocardial infarction and a week after it.
Methods: Serum IL-6 and IL-8 level was measured in 14 patients (8 men and 6 women) with acute myocardial infarction treated with thrombolytic therapy (TT). Blood samples were taken before TT, three hours after it and then on the 1st, 2rd, 3th and 7th day of the disease. There was also measured left ventricle end diastolic diameter (LVEDD), left ventricle end systolic diameter (LVESD) and ejection fraction (EF) on the first day and on the 7th day after MI.
Results:
Serum IL-6 and IL-8 was found to be elevated above the normal values before TT and there was statistically significant increase (p<0,01) in IL-8 level after TT and on the first and second day after MI. IL-6 level after TT was also higher but the increase wasn't statistically significant. IL-6 and IL-8 maintained increased until 7th day after MI - end of the study. There was statistically significant increase in LVESD (p<0,05) and LVEDD (p<0,01) and decrease in EF (p<0,01) on the first day and a week after MI. We found a significant positive correlation between IL-6 level and LVESD a week after MI, between IL-8 and LVESD and LVEDD on the first day after MI and significant negative correlation between IL-6 and EF a week after MI.
Conclusions:

Serum IL-6 and IL-8 are high in the early phase of MI, before TT, what could reflect current inflammatory status.
The increase in IL-8 level after TT could indicate its cardiac origin.
The inflammatory process expressed by an elevated level of IL-6 seems to have an influence on
cardiac function measured by LVEDD, LVESD and EF one week after MI. The same relation applies to IL-8 and LVESD and LVEDD on the first day after MI.
