Patient position:
Supine with the legs extended and stabilized
by the examiner. Patient clasps the hands
behind the head. To test the right external
oblique muscles acting with the left internal
oblique muscles, place the patient in supine
with 45 degrees trunk flexion with the head,
shoulders, and upper torso rotated to the left.
(Note: photograph does not show anti-gravity
test position.)
Examiner pressure:
None. Ask the patient to hold the position. If
the muscles are weak, the trunk will derotate
and extend. The pelvis may anteriorly rotate
and flex on the thighs in an effort to hold the
extended trunk off the table.
Grading:
Since the resistance is varied by the position
of the arms, the grading is as follows:
Normal (5): ability to hold the test position
with the hands clasped behind the head
Good (4): ability to hold the test position with
the arms folded across the chest
Fair (3): ability to hold trunk in enough flexion
and rotation to raise both scapular regions up
from the table.
