A bed-rest patient (part 1)

Abbreviations
Read and listen to the dialogue: (1)
Często pielęgniarki dodają do BR, słówko „complete", aby podkreślić informację, że pacjent nie powinien absolutnie wstawać z łóżka, np: „Patient is complete bed-rest for 24 hours". Powstaje wtedy skrót CBR.
Przekazując informację o aktywności pacjenta pracownikowi medycznemu, pielęgniarka stosuje powyższe nazwy w całości, natomiast pisząc o pacjencie raport pielęgniarski, używa skrótów:
BR - bed rest - pacjent obłożnie chory
BRP - bed rest with bathroom privileges - pacjent z prawem pójścia do toalety
U&A - up and about - pacjent chodzący
RR - Recovery Room - sala pooperacyjna
Conversation: The patient needs bed rest because he's just come from Recovery Room.
Nursing Report: Patient with BR came from RR.
Nurse 1: The patient has bed rest privileges, keep an eye on him. Nurse 2: OK, 1 will.
RN: Hello, Mr. Collins, how is it going today?
Pt: Hello, nurse. I can't complain. I slept all night,
even in the flat position.
RN: What about making your bed and changing all the linen?
Pt: Okay, I'll be glad.
RN: Let's do it. Move closer to the other side of the bed,
so that I can remove the soiled linen and put the fresh one.
Pt: (the patient is turning over) Is it enough?
RN: Perfect. Stay like this for a while and let me replace the bottom sheet and draw-sheet. It's done, now turn back, please. I'm going to the other tide of the bed to tauten the draw-sheet. Hold the bed rails for a
minute. So far, you are doing very well, Mr. Collins. I'm almost done.
Now, you can turn back again.
Pt: It's so comfortable now. I don't feel any wrinkles on my sheet, you did it very accurately, thanks nurse.
RN: You're welcome, Mr. Collins, but I haven't finished yet. Let me replace the pillow cases and the blanket. For your protection, I have to raise the side rails and leave the bed in flat position, as your doctor ordered.
You must stay in this position for 6 more hours; other positions are
contraindicated, as you know.
Pt: I hope, I can stand it, I know this is for my good. Thanks again, nurse.
RN: You're welcome; I'll Ik back in a while.
Assessing a patient:
After the lumbar puncture, the nurse assesses the patient q2hrs for s/s of complications, such as:
Listen and repeat an examplary Nursing Report of Mr. Collins condition: (2)
Collins didn't complain of a headache, didn't vomit either. He was able to e all his extremities. The dressing was not changed yet. No bleeding or any I noted from the puncture side. If he is from pain, he will be discharged a.m.
ability to move his extremities

vomiting, fatigue or anxiety,
condition of the dressing on patient's back.
Patient's dressing may be:
wet,
dray,
blood-stained.

Mr. ( Collins is discharged from hospital and a nurse is giving him advice
about self-care.
This action is called „patient education".
Nurse: Lift your right leg, please. Patient: I'll lift both if you wish!
Read and listen to Patient Education by RN: (3)
RN: Mr. Collins, if you see any fluid flowing out of the puncture side, put a dry dressing on and visit us as soon as possible.
Observe signs and symptoms, or the so-called „aura" of a headache, for example: anxiety or fatigue. If aura occurs, it means that you suffer from migraine. Taking a painkiller when aura is present is the best way to prevent from headaches.
