Abduct the shoulder to 120 degrees. Externally rotate the shoulder so the thumb points to the ceiling. Lift the arm to the ceiling. Examiner pressure: Apply downward pressure against the lateral forearm. Note: the posterior deltoid, teres minor, infraspinatus, and elbow extensors must stabilize the humerus to the scapula in order to use the arm as a lever.
