The influence of cardio-pulmonary bypass and/or aortal clamping on catalase activity.

Background. Increased free oxygen radicals production as a result of ischaemia/reperfusion during coronary artery bypass grafting (CABG) activates free radical scavenger systems including catalase (CT). Activity of CT may correspond the amount of free oxygen radicals production during CABG. In this study we compare three CABG techniques and evaluate its influence on CT activity.

Methods. Study group consisted of 53 patients (aged 42-78 mean 52) who underwent CABG. Patients were divided into three groups. Group A: 19 patients subjected to off-pump procedure. Group B: 18 where anastomoses were constructed on beating heart and systemic circulation was protected by extra corporeal circulation (ECC). Group C: 16 patients ECC with cold crystalloid cardioplegia and aortal clamping. Plasma for analysis was obtained from peripheral artery before ECC installation and before first distal anastomosis (A) in off-pump group. Further samples were taken after 30 min. of reperfusion and 30 min. after last anstomoses in off-pump group (B). Activity of CT was measured on ELISA reader using the method described by Goth.

Conclusion. On-pump CABG procedures result in higher release of CT as compared to off-pump CABG possibly in response to increased generation of free oxygen radicals. Myocardial ischaemia and reperfusion due to aortal clamping results in greater increase of CT activity.
