QT INTERVAL DISPERSION INDICES IN PATIENTS AFTER CORONARY ANGIOPLASTY

QT dispersion (QTd)- marker of heterogeneity of the repolarization of the myocardium- reflects regional differences in that process. Reperfusion (thrombolysis or coronary angioplasty- PTCA) in patients (pts) with acute myocardial infarction (MI) enhances electrical stability of the myocardium.
The purpose of this study was to investigate if PTCA can improve homogeneity of the repolarization in pts with coronary heart disease and which parameters reflect these effect significantly.
The study was performed in 70 pts: 40 pts without the history of MI (nMI) (28 males, age: 55±10 yrs, EF: 62±4%) and 30 pts with previous MI (MI) (28 males, age: 58±10 yrs, EF: 47±10%, 15 pts with anterior MI, 15 pts with inferior MI). Standard 12-lead ECG recordings (50mm/s) were recorded before PTCA (0), 30-60 minutes after (1) and 3-5 days after (2). QT was measured manually and parameters of QT dispersion were calculated: QTd= QTmax- QTmin; QTd'= QTmax2- QTmin2;
QTd"= QTmax3-QTmin3. QTpeak (QTp) and its dispersion were also used.

Both immediately and early after PTCA all indices of the repolarization were lower, significantly for QT dispersion (QTd, QTd', QTd"). These changes were similar in pts with and without previous MI and with anterior or inferior infarction.
Conclusion: Almost all indices shown beneficial effect of PTCA on the homogeneity of the left ventricle repolarization. It was significant both for QTd and for other indices of QT interval dispersion.
