FAOs on Personal Accident Insurance (death) for DSP &PMSP account holders
1) Which is the present Insurance Company for personal accident insurance (death) claims
Ans: Reliance General Insurance Co Ltd
2) Up to which datę the present policy is valid?
Ans: The present policy is valid up to 03.01.2016
3) How I will know the address of the insurance Company?
Ans: The address of die Insurance Company is printed in the death Intimation & claim forms. One has to send die claim related forms/documents to the address mentioned there only. Do not send the papers to any other address of the same company.
4) Where /how I will get the Insurance claim forms?
Ans: The Personal Accident Insurance (death) claim forms have been supplied to the defence/paramilitary HQs for circulation amongst their units. These forms are available at SBI Corporate website: www.sbi.co.in>Personal Banking>Special Salary Accounts>Personal Accident Insurance (death) cover for Salary Accounts. The claim forms are also available at SBI branches.
5) Is natural deadi covered?
Ans: No. Only deaths due to accident are covered. Deaths due to illness are not accidental in naturę. Hence not covered.
6) Are family members covered?
Ans: No. only the Defence/Paramilitary personnel maintaining his/her DSP/PMSP account with us are covered
7) Are joint account holders covered?
Ans: No. Only the DSP/PMSP account holder is covered
8) Does the unit/claimant need to submit the claim documents to SBI branch?
Ans: It is not required. The death intimation and claim forms (along with all relevant documents should be sent directly to the insurance company.
Only die Branch Manager’s certificate (annexure-5) has to be signed from the home branch of the deceased personnel.
9) How the death intimation should be communicated?
Ans. The intimation can also be given through the following channels:

10) Within how many days the death has to intimated to the Insurance Company?
