How to…
Report an Accident
What you need to know
•
Accidents can be prevented.
•
Recording accidents, and investigating their causes,
helps prevent recurring incidents in the future.
•
All accidents to employees, customers, members
of the public, client staff, contractors or visitors must
be reported on the Company Accident Report and
Investigation Form.
•
Every unit must have an Accident Book.
•
Anyone witnessing an accident in the workplace
should complete a Witness Statement.
•
“Near misses”
(i.e. accidents that do not result in an injury
or ill health)
must be logged on an Incident/Near Miss Record.
•
Contact your Line manager or National Health and Safety
Manager if in any doubt about how or which form to fill in.
•
Only appointed persons can complete an accident report.
•
Completed forms must not be left in the accident book,
in order to comply with Data Protection Act.
How we do it…
•
Access the accident book and witness forms and pens
as soon as practical after an accident has occurred.
•
Complete the form as soon after the incident as possible
while memories are fresh.
•
Follow the instructions as detailed in the Accident Book.
•
Fill in the form in detail – an example of a completed
accident form is provided here and can also be found
in the Accident Book.

•
Ask anyone involved in or who observed the accident to complete the witness form in detail.
•
An example of a completed witness statement is provided here.
•
Distribute the document copies as instructed in the Accident Book.
