# 2 - SHIP GENERAL SAFETY
Are you fully updated on the structural condition of the vessel(s) of which you are in charge, are you able to confirm to the Owner / Class that the vessel(s) meet with all Class requirements?
+ Yes
Have the illustrated areas or corresponding area on your vessel been controlled during the past 6 months, and you have proper documentation of such control?
+ Yes
Does your vessel have any structural damage(s) for which Class' recommendation has been issued ?
+ Yes
Have this / these damage(s) been adequately followed up to the extent possible to prevent unexpected development ?
+ Yes
Would high specific gravity cargo pilling up on the tank top affect?
+ The structure of the double bottom, the side check & the transverse bulkheads
What is the purpose of the “Vessel Enhanced Survey File”?
+ All reasons listed under a)to c) above are important with regard to the purpose of the file
When carrying out the Special Survey (Hull) the Owner's representative (the Master) does not inform the Class Survey about defect that he is aware of. In service upon completion of the survey, the defects are resulting in a major accident. Who is responsible for the vessel's condition upon completion of the survey ?
+ The owner, & only the owner is responsible for the condition of the vessel
Why is proper planning of all machinery operation & maintenance activities important?
+ All the above listed statements are important arguments for appropriate planning
Overhaul of auxiliary eng. No.2 is due to take place. A new junior engineer is participating in the job together with an experienced engineer. For the experienced engineer job is routine. Is it important to utilize job descriptions recorded in the Planned Maintenance System (PMS) and Maker's Instruction Manuals ?
+ Yes, it is of almost importance in order to demonstrate proper routines and to document from where information may be called
Why is important to disclose the individual vessel's Machinery Risk Factors ?
+ To ensure safe & efficient operation
On a 5 year old vessel, from what sources would Machinery Risk Factors be available?
+ From all above listed sources
What are the most critical machinery components on board a vessel?
+ Any components of equipment that if fails, is resulting in an immediate lack of power or maneuverability
.Why is it necessary to carry out lifeboat drills every month?(weekly if this is required by flag state)
+ All the above listed statements
2. In case your vessel is involved in busy trade and the port condition restrict the possibility for launching one of the lifeboats. Would there be any risk involved if this boar was neglected over a long period ?
+ There is no excuse neglecting required drills. If drills are not possible to conduct during normal service if the vessel, it is the duty of the Master to suspend the service until the drills has been carried out.
3. What is acceptable if the officer appointed in charge on the lifeboat…?
+ No, that is not OK
Is it acceptable if the officer appointed in charge on the life boat (and him only) is able to operate the lifeboat emergency radio ?
+ No, that is not OK
Would lack of voyage planning, f.inst. outdated navigating documentation, affect the outcome of question of quilt in case of accident ?
+ Yes, neglecting proper voyage planning is in break of SOLAS requirements and is affecting the vessel's state seaworthiness
An experienced officer is signing on a vessel. Immediately upon arriving the vessel, the officer is order to take the responsibility on the bridge. The officer was not checked out on equipment and functions. Is the vessel properly manned ?
+ No, an officer should always go through an appropriate check routine before the take over as OOW
Following statement was logged after a training sequence:” 1400- 1430 hrs officers and crew trained in use of emergency radio equipment”. Is this considered to be appropriate ?
+ It is quite unlikely that all personal could be trained during 30 minutes. Further, no specifications of what particular equipment has been used for training
A container vessel is loaded with containers with general cargo. What environmental risk factors are most likely involved ?
+ All factors related to applicable MARPO Annexes (Annex I/III/IV/V). But also other risk to be considered based on local requirements e.g.: Air pollution, polluted ballast water, refrigerating and fire extinguishing gases etc.
Is the design of a vessel affecting the environmental risk exposure of a vessel?
+ Design criteria may in many connections affect the risk exposure
What type of vessel would have the greatest risk exposure?
+ anyone (either of the ships (two))
